Could you build this?

Partial Wiring existing ASR, LLM and SIP libraries together is feasible with AI help, but real-time telephony audio with low-latency local inference is a hard engineering problem.

What it would actually take: The stack would combine a SIP stack (PJSIP or similar) with RTP audio handling, a streaming voice-activity and speech-to-text model, a local LLM running on MLX or llama.cpp, and TTS, all in a daemon on Apple Silicon. The hard parts are keeping end-to-end latency acceptable, handling barge-in and jitter, and tuning models for telephony audio, which calls for real-time audio and inference expertise.
